Request Sponsorship
OSF HealthCare allocates a budgeted amount of dollars for community support and sponsorships. Sponsorship and charitable contribution considerations are specific to the counties served by OSF HealthCare.
Funds requested should meet the following criteria:
- align with the OSF HealthCare Mission, Vision and Values
- improve access to health care
- promote and/or improve community health.
Sponsorships and charitable contributions will be limited to one time per year for each organization.
If an organization has multiple events in a year, please submit them all together in one request.
Instructions
- If you do not have 2024 version of a W-9 available, download a W-9 form, fill out and save to your computer. The version year can be found in the upper right hand corner of the form. To stay compliant with IRS regulations and ensure accurate tax reporting, we must collect the most current version of the W-9 form (2024). This helps us verify your information, issue 1099s correctly and avoid delays in processing payments. All updated materials, including the completed 2024 W-9 form, must be submitted to be considered. A 2024 version of the W9 form is required. If you do not have 2024 version of a W-9 available, download a W-9 form, fill out and save to your computer. The version year can be found in the upper left hand corner of the form.
- If you are requesting sponsorship of multiple events, download our multiple event request form, fill out and save to your computer.
- Complete the sponsorship request form below. Make sure to upload your 2024 W-9 and, if applicable, multiple event request form, in the appropriate field.
- We will review each request and contact you via email.
