What about insurance coverage?

Your insurance might cover services from OSF HealthCare. Check with your insurance company to determine if OSF is an in-network provider.

We offer menstrual cycle charting instruction to help you and your partner achieve or avoid pregnancy and monitor gynecological health. If a fertility-related diagnosis is made, we will bill your insurance for associated medical services, including Medicaid. You are responsible for deductibles and copays. 

The coverage for vasectomy reversal depends on your insurance company, so it’s a good idea to reach out to them for details about your benefits before you visit.

What do I need to bring to my appointment?
